non public parent reimbursement request forms
Iowa Code (Section 285.1) requires public school districts to provide transportation services to resident nonpublic students that are entitled as per that section (please see “Notice to Nonpublic Parents” on the reimbursement form for more details).
It is the parent/guardian’s responsibility to submit a request for nonpublic parent reimbursement for travel to and from school. For elementary students (K-8), you must live two (2) miles or more from the school to quality; for secondary students (9-12), you must live three (3) miles or more from the school to qualify. Forms must be completed, signed, and submitted to the appropriate school district address (near the bottom of the form) by December 1st (for first semester reimbursement) and May 1st (for second semester reimbursement), each year. Failure to submit this request by the above deadline each semester will result in the denial of the reimbursement request for the claim period.
Reimbursements are always a semester behind. For example, 1st semester reimbursements are usually not received by the parent until mid to late spring of that school year; 2nd semester reimbursements are usually not received by the parent until mid to late fall of the next school year. Any questions regarding receipt of the mailed forms or reimbursements should be sent directly to your resident school district.
